Our client is a leading healthcare provider specializing in post acute continuum of care through comprehensive homecare and rehabilitation programs. We are currently seeking a RN Team Leader in Fayetteville, AR.
As a RN Team Leader, you will...
- Initiate and coordinate patient's plan of treatment and care.
- Provide evaluation of patient's condition and communicate information to the physician, staff, therapists and payer sources.
- Coordinate transfer and discharge of patients.
- Conduct chart audits of quality assurance and utilization review and report findings to the DON and Branch Manager.
- Assist DON and Branch Manager in planning, implementation and evaluation of in-service and continuing education programs for staff.
- Assist in strategic planning and evaluation of policies and procedures.
- Associate’s degree in Nursing or the equivalent.
- Current RN license in the State of Arkansas.
- Minimum of 2 years home health nursing experience.
- RN Case management experience required with current OASIS experience.
- 1 year of Home Health supervisory / management experience required.
- Strong leadership, organizational and communication skills.
- Ability to work effectively in a team environment.
Salary - $52 - $55K
