Our client is a leading healthcare provider specializing in post acute continuum of care through comprehensive homecare and rehabilitation programs. We are currently seeking a RN Team Leader for Home Health in Centralia, WA.

In this role, you will...

Coordinate the efforts of a team to assure continuity of high quality care to home health patients.
Case manage a team.
Troubleshoot with physicians and obtain orders.
Conduct audits of quality assurance and utilization review and report findings to Branch Manager.
Assist Branch Manager in planning, implementation and evaluation of in-service and continuing education programs for staff.

Requirements:

Associate's degree in Nursing or equivalent.
Minimum of 2 years experience home health experience.
Current RN license.
Recent Medicare home health experience.
Recent OASIS Assessment skills.
Ability to multi-task and work effectively in a fast pace team environment.

Strong leadership and communication skills.

Salary ($58-$65K)
